The Real End is an achievement on the Xbox, Steam/PC, and PSN versions of Super Meat Boy. To unlock it the player needs to defeat Dr. Fetus again but this time in the Dark World.

As long as you know how Dr. Fetus's Dark World boss fight works you should be fine it will be harder though since you're in the Dark World.
